さて、トクホについての記事で、興奮して書いたと思われる記事を再掲載する。

反論:「新潮」トクホの大嘘
2017年3月30日の「週刊新潮」で、多くのトクホで、有効であるという研究自体に問題があり、科学的根拠が乏しいにもかかわらず、効果があるとして販売されていることが記された。
これまで私が主張していたことではあるが、今回、逆にトクホ側に立って、ディベートし、反論してみる。
1. 1)難デキストリンの入った茶(A)と入っていない茶(B)を比較した実験で、Aではきつねうどんとご飯、Bでは大盛りのカレーライスを食している。そのような実験で血糖が抑制されたとすることは意味があるか?
<反論>確かに、うどんは小麦粉なので、小腸から吸収されないため、それ自体、血糖は上昇しにくい。しかし、その後に血糖は上昇するであろう。つまり、大盛りのカレーライスを食べた方が血糖は早期に上がりやすい。であるから、食事の違いによる血糖上昇の違いであって、難デキストリンの入った茶が血糖上昇を抑制したかどうかはわからないが、そのような極めて基本的なバイアスの問題のある研究は、きちんとした査読者にチェックされるジャーナルでは却下されるには違いない。しかし、そのようなきちんとした査読のあるジャーナルに掲載されているわけではないのであるから、そもそも、科学的な批判には値しない。
2. 2)難デキストリンを摂取した群で、糞便の脂肪が2倍多いという結果から、難デキストリンは脂肪の吸収を抑制するとしているが、摂取量から換算すると、吸収された脂肪の差はわずか1.2%にすぎない。
<反論>確かに、吸収される脂肪の量はほとんど変化しないかもしれないが、排泄される糞便の栄養成分を分析して、どれくらいの栄養が体内に吸収されるかという研究は何十年前に行われた歴史的な研究方法である。そして、1.2%であっても、たとえそれはわずかだとしても、たとえ、統計学的に有意差がなくとも、「脂肪の吸収を抑制する」というのは虚偽や嘘ではない。「嘘」とは意図的にデータを捏造する場合の用語であり、意図的ではないデータの誤解釈は「誤り」「間違い」であって、嘘ではない。
3. 3) 糞便の脂肪量から推定される脂肪吸収抑制量と実際に測定された胴回りの脂肪減少率とは乖離している。難デキストリンの作用機序に矛盾があり、騙されている「被害者」いかに多いか。
<反論>脂肪吸収率が低下して、それが全て胴回りに影響するとは限らない。しかし、わずかであっても胴回りの脂肪が低下することが重要である。そもそも、他国から疑問視されていたにもかかわらず、胴回りの測定によるメタボリックシンドロームという日本の診断基準が国に認定されたことが重要であり、その診断基準によって、日本人は胴回りについては非常にナーバスであり、それに対するトクホの消費は日本経済に有益である。つまり、詳細な科学的分析よりももっと重要な経済的な意味を持っている。(「騙す」「被害者」については、反論6を参照)
4. 4) ケルセチン配合体の入った茶での実験では、入っていない茶を摂取した群では明らかに摂取カロリーが多い。
<反論>反論1.と同様

5)人工甘味料を安全とした論文は、企業から研究費を支給された研究機関が書いたものであり、企業と関係ない機関からの90論文の9割は脳腫瘍などの危険を指摘している。
<反論>これは、人工甘味料に限らず、ひとつの否定的な論文が出ると、その何倍もの有効性を示す論文が企業から発表されるというのがこの業界の常識である。日本で使用されている人工甘味料は国が安全性を吟味して認可しているものであり、病気の発症との関連性が明らかになれば、使用は禁止されるに違いない。つまり、結局、その責任は国にあり、企業や研究者の責任ではない。
6. 6)腸内フローラはバランスが重要であり、一種類の乳酸菌を増やす効果は疑問である。
<反論>総じて、科学的にトクホに効果があるか、効果がないかと論じているが、科学的効果がなくとも、「効果があると信じて摂取する」と「効果がないと思って摂取する」よりも効果が得られる。それをプラセボ効果というが、このプラセボ効果がトクホでは重要なのである。要するに、科学的効果などなくてもいいのである。「効果がある」と思い込むことが、何よりも有用なのである。そのプラセボ効果が国としての経済効果(消費税など)を得るのであれば、国として利益があるのである。さらに、トクホの記載によって、日常的に食品の選択に気を配る「啓蒙的」効果も期待されるのである。

Abstract
BACKGROUND:
As the physiological functions of soluble dietary fibre, the favourable efficacy, such as attenuating the absorption of saccharides or lipids, is expected. Resistant maltodextrin, a soluble dietary fibre, was investigated and found that it delays the glucose absorption and attenuates the postprandial rise in the blood glucose levels, however, the efficacy of resistant maltodextrin on lipid metabolism is not yet reported.
AIM OF THE STUDY:
We conducted an animal experiment and a human experiment to investigate the effect of resistant maltodextrin on postprandial blood triacylglycerol elevation.
METHODS:
1. Rats were fed corn oil with or without resistant maltodextrin and the postprandial changes in triacylglycerol were examined. 2. We then conducted a dietary loading experiment on 13 healthy adult male and female subjects using a meal containing approximately 50 g fat. A beverage not containing resistant maltodextrin was used as a placebo; subjects consumed the loading meal and a beverage containing either 5 g or 10 g resistant maltodextrin; blood was periodically collected to see the changes in serum constituents.
RESULTS:
1. The corn oil administration experiment using rats showed that resistant maltodextrin dose-dependently suppressed elevation of blood triacylglycerol levels after corn oil administration. 2. The dietary loading experiment on 13 healthy subjects with 5 or 10 g of resistant maltodextrin showed that; in each administration group, resistant maltodextrin significantly suppressed postprandial elevation of blood triacylglycerol, RLP-cholesterol and insulin.
CONCLUSION:
These results indicate that resistant maltodextrin ingested with fatty meals suppresses the postprandial elevation of blood triacylglycerol levels.

Effect of Resistant Maltodextrin on Digestion and Absorption of Lipids

In order to confirm the mechanism how postprandial elevation of triacylglycerol is suppressed by resistant maltodextrin (Fibersol-2), we conducted the following experiments. Firstly, Rats were fed a high-fat diet with resistant maltodextrin at 0% (control), 2.5% or 5% for 5 weeks to determine the lipid amount excreted in the feces of the last three days. The total lipid and triacylglycerol excreted in rat feces were significantly increased in a dose-dependent manner by the ingestion of resistant maltodextrin. Secondly, 10 healthy adult subjects were administrated a beverage containing 5 g resistant maltodextrin or placebo for 10 days, and crossed over after an 11-day washout. Total lipid excreted in feces was determined by collecting all the feces for the last three days. Fecal weight and fecal lipid amount of the subjects increased significantly with the ingestion of resistant maltodextrin compared to the placebo ingestion. Thirdly, oil emulsion prepared with resistant maltodextrin was assessed for its hydrolysis rate by lipase. The hydrolysis rate of lipid by lipase was not affected by resistant maltodextrin. Lastly, micelle emulsion was prepared with or without resistant maltodextrin, and their stability was compared. Resistant maltodextrin inhibited the decomposition of micelles and stabilized micellar structure. From these results, it was suggested that resistant maltodextrin suppresses lipid absorption and promotes the excretion of lipid into feces by delaying the release of fatty acids from micelles in the lipid absorption process. No inhibitory effect on lipase activity was observed by resistant maltodextrin.

Improvement effect of resistant maltodextrin in humans with metabolic syndrome by continuous administration.

Abstract
Resistant maltodextrin (RMD) is a soluble dietary fiber ingredient whose physiological functions are well recognized in Foods for Specified Health Use (FOSHU) for maintaining healthy intestinal regularity, blood glucose levels, and serum lipids. However, its efficacy on combined health risks--metabolic syndrome--was not studied yet. In this study the efficacy of RMD on humans with metabolic syndrome was investigated. A randomized double-blind placebo-controlled parallel-group trial was conducted. Thirty subjects with metabolic syndrome were randomly allocated into 2 groups and took either tea containing 9 g of RMD (treatment group) or placebo tea at three mealtimes daily for 12 wk. Blood was collected and body fat was scanned periodically. In the RMD treatment group, waist circumference, visceral fat area, fasting blood glucose, HOMA-R and serum triacylglycerol (TG) were significantly decreased compared to baseline, and significant time-by-treatment interaction was observed for waist circumference, visceral fat area, HOMA-R and serum TG (p=0.044, p=0.012, p=0.032, and p=0.049, respectively). The change ratio of visceral fat area showed negative statistical correlation with the baseline value (p=0.033), suggesting that efficacy of RMD was emphasized in the subjects having a larger visceral fat area. After the 12-wk RMD treatment, the total number of metabolic syndrome risk factors decreased to 20 from 32 with 2 subjects having no risks, while that of the placebo group decreased to 25 from 32. These findings suggest that continuous ingestion of RMD may improve the risk factors of metabolic syndrome by reducing visceral fat and improving glucose and lipid metabolism.
<有用性>
- 研究デザインがランダマイズド・ダブルブラインド・プラセボコントロールである。
- 12週投与で、プラセボと比較して腹囲が約2㎝減少し、内臓脂肪も低下した。
- 下痢はあったが、特異的でない。
<問題点>
- 腹囲が減少したが、95から93㎝であり、メタボのままである。
- 体重とBMIが減少していない。
- 内臓脂肪は低下したが、皮下脂肪が増加している。
- 空腹時血糖、空腹時インスリン、LDLコレステロール、レプチンの変化はプラセボと同じ。
- 腹部症状(放屁増加と腸音増加)が30%に出現した。

<追記>
トクホで問題なのは、今回記された動物実験や健常者での研究の不正確さではない。
トクホで問題なのは、
動物実験や健常者だけの研究しか行われていないことにある。
トクホは、健常者での有効性と最高投与量が検討されているが、対象は健常者のみであり、
何らかの疾患を有する場合の有害作用を考慮していないことが問題なのである。
具体的には、過敏性腸症候群では、「お腹の調子をととのえる」というトクホの成分のなかで、発酵性糖質は疾病発症の因果関係が問われおり、症状悪化の因果関係は明確になっている。さらに、潰瘍性大腸炎、クローン病、大腸憩室でも因果関係が示されている。
よく、トクホは医療費削減のための健康増進を目的とするともいわれる。
しかし、過敏性腸症候群は、日本では推計1700万人存在する。
日本国民の1700万人の症状を悪化させるのであれば、医療費削減という言葉はあてはまらない。
食品だから薬と違って、副作用など考える必要がないと、もし、思っているのであれば、間違いである。
過敏性腸症候群であることを知らずに、お腹の調子が悪いために、「お腹の調子をととのえる」というトクホを摂取する人は少なくないことが問題なのである。
トクホにかかわっている人々は、国民の健康のために貢献しているとプライドを持って頑張っているであろう。
しかし、浅学によって、特定の人の病状を悪化させている可能性がないとは言えないことを知らねばならない。
以上から、トクホでは、
摂取してはいけない疾患を正しく国民に告知しなければならない。
