「日本のピロリ菌除菌戦略の失敗」という論文は

2018212日に論文が完成した。

その内容は、

「ピロリ菌除菌によって、胃癌が減少することが出来るとして、除菌推進者は、国費を投入してすべてのピロリ菌陽性者の除菌治療を国費で行うべきだと主張し、国は慢性胃炎の除菌療法を承認した。しかし、その後、彼らの主張の根拠となる研究が否定され始めた。さらに、推進者の予想の通りに胃癌は減少していない。さらに、除菌後の副作用が懸念される」

というものであった。

その論文は、世界の医学者の意見を聞くために、

世界の医学雑誌に送り続けた。

連続する不採用にも関わらず、とにかく、世界の意見を聞きたかったのである。

合計13の医学雑誌に送られ、13回不採用となった。


しかし、目的は達成した。

何十人もの世界の医学者の脳裏に定着させることが出来たであろう。

そして、多くの意見を聞くことが出来た。

This study elaborated the profitability of Janpan's strategy for eradicating Helicobacter pylori to prevent gastric cancer. 

The pathogenesis of gastric cancer is thought to occur by the Correa pathway (precancerous cascade) as the transition from normal gastric epithelial tissue to invasive cancerous tissue via inflammation. In addition, the infection of Helicobacter pylori is defined as a human class I carcinogen. Therefore, eradication of Helicobacter pylori to prevent the occurrence of gastric cancer is generally recognized.

However, there are still a number of issues which need to be addressed.

1.Although the eradication of Helicobacter pylori can reduce the incidence and mortality of gastric cancer to some extent, the side effects of therapeutic drugs should also be brought to our attention. Such as antibiotic allergy or resistance, Clostridium difficile infection, fracture, pneumonia and renal dysfunction caused by PPI. On the other hand, there is risk of reinfection after eradication, so it seems unreasonable to eradicate asymptomatic children.

2.The incidence of gastric cancer decreased after eradication of Helicobacter pylori, may due to the atypical appearance of gastric cancer under endoscopy after eradication, which may lead to missed diagnosis. Therefore, it may exaggerate the role of eradication of Helicobacter pylori in reducing the incidence of gastric cancer.

この研究は、胃がんを予防するためにヘリコバクターピロリを根絶するためのJanpanの戦略の収益性を詳しく述べたものです。

胃癌の病因は、正常な胃上皮組織から炎症を介した浸潤性癌性組織への移行としてのコレア経路(前癌性カスケード)によって起こると考えられている。さらに、ヘリコバクターピロリの感染はヒトクラスI発癌物質として定義されている。したがって、胃癌の発生を予防するためのヘリコバクターピロリの根絶が一般的に認識されている。

2.胃癌の発生率はHelicobacter pyloriの撲滅後に減少したが、これは根絶後の内視鏡検査下での胃癌の異型性の出現による可能性があり、これは誤診につながる可能性がある。したがって、それは胃癌の発生率を減らすことにおけるヘリコバクターピロリの根絶の役割を誇張するかもしれません。

The paper is based on a review of recent studies related to H.p. eradication strategy, trends in the diagnosis of gastric cancer and almost early gastric cancer, the burden of upper G.I. endoscopies.

In the matter of fact, it's difficult for me at present to establish whether the decline in the number of gastric cancer diagnosis in Japan is related to the H.p. eradication claimed from 2013 or to the increased number of upper g.i. endoscopies performed lasting the years.

This appears to be a review study/letter arguing a case that recent declines in gastric cancer mortality are due to gastroscopy screening and that increased rates of H pylori testing and treatment have not contributed to this. I find the argument difficult to follow, and I think the author ignores the most likely explanation that the reduced mortality is likely due pre-existing trends of declining H pylori prevalence in the population. Furthermore, none of these arguments appear to be relevant to whether a test and treat program in asymptomatic population may be an effective and cost-effective way of reducing gastric cancer incidence and mortality.

 

Major points

It is difficult to get a sense of what clinical practice existed before 2013 and how it changed thereafter. Are all adults offered gastroscopy? Was the new practice to test people who had gastritis for H. pylori and treat them if they were positive? Of note, it is extremely difficult to assess the impact of a screening programme in clinical practice without a randomized controlled trial. So to make bold claims based on the included data is unhelpful.

It would be useful to have some stronger comparisons with practice in other countries and how relevant this discussion is to elsewhere (I note the US is mentioned).

It is expected that eradication before gastritis is detected could be more beneficial for stomach cancer prevention than waiting for these signs or symptoms and then testing and treating, when the pathway to gastric cancer may already have begun. The evidence for H pylori eradication reducing stomach cancer is strongest for testing and treating asymptomatic people in high risk populations, with the most relevant paper being eg Ford et al 2014 meta-analysis - but this paper is not referenced here citing Yoon instead which is about H pylori treatment after gastric cancer surgery.

The point the author makes about the difference between RRs vs RDs is not relevant, both should be significant.

I do not understand logic in the argument about cost-effectiveness and it may be flawed. If endoscopy is already funded and in place with follow-up procedures for gastritis, then the main marginal additional costs of testing and treating for H pylori are as quoted the cost of the test and the treatment.

The Japan system of endoscopy screening is particularly resource intensive, and occurs in very few countries. I would like to see a discussion about why this practice is in place and the evidence for its effectiveness and cost-effectiveness. This is a separate discussion from the one about using H pylori eradication in patients with gastritis who are tested positive.

The final argument and conclusion was unclear, and should be reiterated in the abstract.

Are patients who are H pylori +ve also retested and what is the eradication rate for those who are treated?

The author says that Japan follows Pattern B2 but I could not find any evidence for this. Should this reference a figure?

Circumstantial evidence is quoted but not critiqued about the association of eradication with Crohns, Barrett's esophagus, esophageal adenocarcinoma etc.  I think the evidence for these was unclear/weak.

これは最近の胃がん死亡率の低下が胃内視鏡検査によるものであり、ピロリ菌検査と治療の増加率がこれに寄与していないという事件を論じるレビュー研究/手紙のようです。私はその議論をたどるのは難しいと思います、そして著者は死亡率の低下は人口の中でのピロリ菌の有病率の減少という既存の傾向が原因であるという最も可能性の高い説明を無視すると思います。さらに、これらの議論はいずれも、無症候性集団における試験および治療プログラムが胃がんの発生率および死亡率を減少させるための効果的かつ費用対効果の高い方法であり得るかどうかに関連するとは思われない。

In this manuscript, Uno tries to convince the readers that the policy of Helicobacter pylori eradication promoted by the Japanese government since 2013 is not worth pursuing.

Even if some of the arguments proposed are correct, the interpretation of the author is not valid.

This reviewer could agree that to see a dramatic decrease in gastric cancer incidence and mortality, a minimal period of 10 years is necessary.

The case of H. pylori infection and gastric cancer can be compared to the case of smoking and lung cancer. It was shown that after smoking for several decades, to quit smoking led to a progressive decrease in lung cancer occurrence , but to reach the basal level of lung cancer of those who never smoked, more than 20 years are needed (Kenfield et al JAMA 2008;299:2037-47).

Indeed, despite the disappearance of the villain by eradication, following decades of infection, mutations may remain in local cells which will lead to gastric cancer in some patients. So metachronous gastric cancer may occur but the frequency will decrease over the years, and an endoscopy follow-up is not required every year as stated by the author, which will decrease the cost of the strategy.

In this respect, long term follow-up (>10 years) has shown that even intestinal metaplasia, a premalignant condition, could regress after H. pylori eradication, even if the definite proof using gastric cancer as the outcome measure is not yet available. So, it is not a surprise that the Japanese government and pharmaceutical company did not guarantee the effectiveness.

It is also wrong to say that the decrease in H. pylori infection was the consequence of the development of flush toilets in the country. This would imply that this bacterium is essentially transmitted by the faecal-oral pattern, which is not the case.

On page 18, the author embarks in defending the supposedly negative aspects to be H. pylori free. Even if H. pylori is part of the so-called hygiene hypothesis, it is now shown that it is a lack of diversity of the microbiota and lack of exposure of the immune system to a variety of antigens which may lead to chronic diseases.

So, in summary if the policy of H. pylori eradication in gastritis has overestimated the immediate benefits of this action, it is clear that this policy will definitely have a positive impact on the mortality due to gastric cancer in Japan, compared to waiting for a natural disappearance of the bacterium following improved socioeconomic conditions, and it would be a terrible mistake to stop it.

以上のように、この論文は、どこにも掲載されていないにもかかわらず、大いに議論を生み出した。
本来、改定後採用となるようなレビューの意見であっても、
全てのジャーナルの責任エジターは、アクセプトしたという責任を恐れ、不採用とした。

しかし、今年の目標は達したのである。

この論文は、以上の指摘を得て、より完全なものになって、来年、リニュアルして、ふたたび、世界中のジャーナルに送られる予定である。

その際には、最新のデータに切り替え、これまで1800億円を投資してピロリ菌除菌数が拡大されたにもかかわらず、経過とともに、除菌後胃癌が増加し、そして、ついに胃癌死亡の減少傾向が止まったことを記載する予定である。

This paper presents an interesting account of Japanese governmental decisions relating to the licensing of H. pylori eradication therapy for the purpose of gastric cancer prevention. It makes fascinating reading as a piece of medical history/sociology. I am not sure to what extent the paper represents a contribution to scientific research.

1.      The paper would be considerably improved if it were thoroughly edited by a native English speaker. In places the narrative is difficult to follow and I think much of this is due to inadequate language.

2.      A serious issue, if the paper is to be taken as a scientific contribution, is some understanding of the extent to which the author is acting as a reporter of deliberations and decisions within the Japanese Ministry of Health, Labour and Welfare based on his review of governmental papers and the extent to which he has identified and reviewed the relevant literature. If the latter, is the author to what extent is he assuming that this represents the evidence base underlying government decision making. In other words is the author “telling the story” or is he providing evidence to contribute to the story. Both are legitimate but, as written, it is difficult to understand the author’s role. The paper requires a clear statement about what is its intended aim.

3.      The Methods section of the paper is very short (7 lines) and this contributes to my concerns expressed in point 2 above. The author implies he carried out a systematic literature review but how many papers were identified, did he only consider Japanese studies, what criteria did he use to include a paper in his review and did he really only identify and consider  “negative” studies (what happens to positive studies). The author fails to indicate the sources or methods used for analysis incidence and mortality trends or how “expected” values were derived.

4.      As I understand the flow of the paper, the author implies that the basis of the government’s current position is that i, there are now several studies contradicting the results of the influential Fukase 2008 paper on MGC (alongside concerns about the identification of MGC) ii, the timescale to detect an influence of eradication on incidence and mortality was initially underestimated; iii, the costs of an eradication strategy are considerably more than originally estimated; iv, the anticipated decreases in incidence and mortality have not been observed; v, no consideration has been given to detrimental medical side-effects of population-based eradication. If this is correct, the paper could usefully be broken down to consider each of these elements in turn while, perhaps deleting some of the superfluous detail (e.g. how the authorship order was determined for Fukase 2008).

本稿では、胃がん予防を目的としたピロリ菌根絶療法の認可に関する日本政府の決定に関する興味深い説明を提示する。それは病歴/社会学の一部として魅力的な読書をします。この論文が科学研究への貢献をどの程度表しているのかは定かではありません。

There are multiple individual studies from Asian countries showing a benefit of H. pylori eradication in asymptomatic adults and a subsequent reduced risk of gastric cancer. The Japanese authorities made the decision for this strategy based on the best available evidence. It is unsurprising that this came with a cost (in financial terms) - and it would be wrong to consider that "unexpected". Furthermore, it is premature to conclude that the strategy is ineffective since it has only been in operation sicne 2013.

世界は、日本の戦略の今後の結果を注視している。

つまり、国家レベルで、ほとんどすべてのピロリ菌陽性に対して除菌をした後の結果を知りたがっているのである。

世界にとっては、この戦略に日本の税金がどれだけ投入されようと、赤字が見込まれていても、そんなことはどうでもいいのである。お金があるのであれば、どんどんやってくださいということである。

私が、日本が今、「大実験場」となっているというのは、その意味である。

そして、それに対して、異議を唱える一人の日本人もいないのであれば、この国はあまりにも、悲しい。

その意味で、私の活動は、意味があるに違いないと信じている。
