FODMAP食:デンマークと豪州の論争

 

20172月に公開されたデンマークの過敏性腸症候群(IBS)の診断と治療ガイドラインにおいて、

FODMAP食の有効性が記され、そのエビデンスレベルは、1bと評価されている。

この評価は、そのガイドラインでは高評価に値する。

A so-called Swedish IBS diet is less invasive and can reduce symptoms as effectively as LFD (LFD: 22% symptom reduction and 50% responders vs. 24% symptom reduction and 46% responders).

Abstract

The low FODMAP (Fermentable Oligo-, Di- and Monosaccharides and Polyoles) diet (LFD) allegedly reduces symptoms of irritable bowel syndrome (IBS). Eleven studies have examined the effects of LFD on IBS. Most studies reported a symptomatic effect, but methodological weaknesses such as lack of relevant control group and of proper blinding means that a placebo response cannot be excluded. No studies have examined the effect of the important reintroduction phase nor the effects of LFD on IBS patients in primary care. Evidence suggests that intake of high dose FODMAP can induce gastrointestinal symptoms, but the clinical relevance of this is doubtful.

しかし、20176月、英語で低FODMAP食に対する批判論文が掲載された。

Abstract

BACKGROUND:

The low Fermentable Oligo-, Di- Monosaccharides, and Polyoles (FODMAP) diet is a new treatment option for irritable bowel syndrome (IBS). Experts refer to the diet as supported by high level of evidence, but an evaluation of the quality of trials is lacking.

AIM:

To provide a systematic review of the quality of trials on the symptomatic effects of the low FODMAP diet for IBS.

METHODS:

Pubmed and EMBASE were searched for randomised controlled trials (RCTs) reporting effect of the low FODMAP diet on IBS symptoms. The quality of trials was evaluated by estimating risk of bias and assessing trial methodology.

RESULTS:

Nine RCTs were eligible, including 542 patients. The intervention period was from 2 days to 6 weeks and one trial included a 6-month follow-up. Three trials intervened by providing meals, controlling with a diet high in FODMAP content. In six trials, the intervention was instruction by a dietician and a variety of control interventions were used, all with limited established efficacy. Domains with a high risk of bias were identified for all the trials. High risk of bias dominated domains regarding blinding, with only one trial double-blinded.

CONCLUSIONS:

The RCTs on the low FODMAP diet are characterized by high risk of bias. The diet has not been studied in a randomised, controlled setting for more than 6 weeks and trials examining the effect of the important reintroduction period are lacking. There is a risk that the symptomatic effects reported in the trials are driven primarily by a placebo response.

結果:RCT9件、542件が対象となった。介入期間は2日から6週間であり、1回の試験は6ヶ月のフォローアップを含んでいた。食事を提供する3つの介入試験があったが、FODMAP含有量が高い食事をコントロールしていた。 6つの試験では、介入は栄養士による指示であり、様々な制御介入が用いられ、すべて有効性が確立されていた。偏見のリスクが高いドメインはすべての試験で同定された。盲検化に関するバイアスの多い領域のリスクが高く、唯一の試行は二重盲検である。

結論:低FODMAP食のRCTは、バイアスのリスクが高いという特徴がある。ダイエットは無作為化された制御された環境で6週間以上研究されておらず、重要な再導入期間の影響を調べる試験は欠けている。この試験で報告された症候性の影響は、主にプラセボ反応によって引き起こされるリスクがある。

In the critical review of the quality of low FODMAP diet trials in irritable bowel syndrome (IBS),[1] several valid points of weakness are highlighted. However, bias in the critique can also derive from factual inaccuracies. We offer three of several examples.

In a harsh criticism of the pivotal double-blinded, cross-over study in which nearly all food was provided,[2] the authors stated that the diet used as placebo control was really a “high FODMAP” diet. We do not agree. Increases in FODMAP intake over that in the habitual diet (retrospectively assessed) were marginal. Moreover, symptoms whilst on the control diet were not increased over those on the habitual diet in either IBS patients or healthy controls.

Secondly, it was suggested that discussion of a short intervention designed to compare intestinal gas production on high or low FODMAP diets[3] had “interpretative bias” because it “failed” to discuss that “gas production was not associated to induction of symptoms”. This was despite the fact that this association was indeed present in the patients with IBS, but of course not in healthy subjects. Who was guilty of interpretative bias?

The success of blinding in a cross-over study cannot be assessed by asking subjects to identify their dietary arm on completion of the study when there is a marked difference between symptoms induced in each arm. In fact, the ability of subjects to guess the identity of the therapies in blinded cross-over studies has been used as a primary end-point.[4] Because subjects with IBS identified which diet was low in FODMAPs in the pivotal feeding study,[2] the authors believe this means failed blinding. We strongly disagree as outlined in the paper; the subgroup (healthy subjects) who had no symptomatic cues were unable to correctly guess the nature of the two dietary arms.

The authors appear to have their own bias towards the current “standard dietary recommendations” (in their view, the NICE guidelines for the dietary management of IBS), but there is minimal acknowledgement that this is not evidence-based.

The authors did introduce a positive note by suggesting a trial design in which a low FODMAP diet is compared with linaclotide in IBS-C patients. However, the double-dummy design is no solution to the issue of the design of a placebo diet and the restriction to patients with self-reported constipation seems counterintuitive to the concept of the low FODMAP diet.

Does the potential bias introduced by putative weaknesses mean that the published data are of minimal value as implied by Krogsgaard et al.? The efficacy of any therapy is judged not purely on the basis of individual randomised controlled trials. It is judged on the entirety of the data and on ‘real-world experience’, which, for therapies that are very challenging to study in the most stringent way (such as dietary studies), is very important in informing clinical practice. Taking a prescriptive and negative view of studies reported blurs the overall message of the studies that a low FODMAP diet works.

弱い推定によるバイアスの可能性はKrogsgaardらによって暗示された最小値であることを意味するか?いずれの療法の有効性も、個々の無作為化比較試験に基づいて純粋に判断されるものではない。

We thank Gibson et al. for their letter[1] regarding our review.[2] Gibson and co-workers argue that in the study by Halmos et al.,[3] the increase in FODMAP content is marginal in the control diet (23.7 g/d, 95% CI: 16.9-30.6) as compared to the baseline diet (16.3 g/d, 95% CI: 14.1-18.5) and that the control diet should not be classified as a high FODMAP diet. We do not agree that the increase in FODMAP content is marginal and, even though confidence intervals are overlapping, we are concerned that the higher FODMAP content in the control diet may have influenced symptom reporting. Indeed, data reported in the paper showed a highly significant difference in overall gastrointestinal (GI) symptoms measured on a 100 mm visual analogue scale between baseline and the control diet (36.0 mm vs 44.9 mm, P<.001).

Ong et al.,[4] reported that patients with irritable bowel syndrome (IBS) had a higher production of hydrogen but not methane on a high FODMAP diet compared to a low FODMAP diet. Furthermore, it is reported that, “There was no association of the pattern of hydrogen and methane production with the induction of symptoms”. Therefore, we do not agree with Gibson et al. that the association of gas production to induction of GI symptoms was indeed present in IBS patients.

Regardless of the study design (cross-over or parallel) it is relevant to uncover possible unblinding by asking patients if they can identify the active treatment.[5] Symptom response is one factor among many that may unblind patients, but it is not possible to conclude if symptom response lead to unblinding or—on the other hand—if unblinding influenced symptom reporting. As mentioned in the discussion paragraph in our paper, many results from the various treatment trials, including lack of consistency between subjective reporting and objective measures, the almost universal effect on all GI and general symptoms and the variation in the time to symptomatic response, suggest that a placebo response, made possible by unblinding, could be the driving force behind the positive outcomes reported from the trials.

We agree that standard dietary recommendations for IBS, including the NICE guidelines, are based on limited evidence. In fact, we stated that both in the results section and in the discussion of our paper.[2]

Our systematic review was performed to focus on the limitations of the studies conducted on the low FODMAP diet, as this new treatment for IBS is often appraised as being supported by good evidence. What should be deducted from our review is not that the published data are of minimal value, but that we do not yet have evidence to conclude that the low FODMAP diet is an effective treatment for IBS. Finally, we definitely acknowledge the many challenges regarding both trial design and execution of trials on the symptomatic effects of a diet. We maintain that diary therapy should be subjected to the same methodological rigour as pharmaceutical products that claim to provide long-term benefit in IBS.

要するに、デンマークの研究者は、

FODMAP食がIBS症状を改善する可能性を認めるが、

その研究方法に若干問題があることを指摘し、まだまだ、多くの研究が必要であるということである。

そして、私も(デンマークの研究者と重複する内容もあるが)、以下が問題だと思っている。

 

プラセボ研究が十分か?

ガス増加と症状の関連性の明確化が必要

FODMAP食と比較された食事が高FODMAPすぎないか?

長期的に症状再発した場合はどうするか?

 

これに対して私は以下と考える。

プラセボ効果:効果があると思うだけで、体内で何らかの変化が起こり、良い結果が得られることは実際に証明されている。そして、逆に、その治療が嫌だと思うだけで、効果が無くなる「ノセボ効果」というものもある。実際に、何の治療も信じられないと思い込んでいる人は、どんな治療も有効になりにくい。低FODMAP食は、好きなものが食べられない!と思うだけで、ノセボ効果によって、効果が無くなる可能性もある。このような非実質的効果を除外する研究は、食事療法ではある方法で検証可能である。これについては、現在、Gastroenterology に投稿中である。

ガス増加による症状:これは、これまで私の最大の課題であったが、これについても現在、投稿中のもの論文と、また、公開予定の論文がある。

FODMAP食と比較された食事が高FODMAPすぎないか?:これについても、間接的に私の意見は投稿中である。

長期的に症状が再発した場合はどうするか?:これについては、長期的に再発したというのは、厳しい制限食で一旦、良くなった事実があり、チャレンジに失敗した可能性があり、再び導入期を行う必要があると考えている。

いずれにせよ、歴史的に、論争なき研究は非常に怪しいものである。
論争が起こって、そして、それらをクリアしてゆくことが正しい研究には必要不可欠である。
そして、低フォドマップ食に関しては、私の頭の中では、全てクリアしているが、
なにか課題がないか、探している。
