In Rome IV, patiemt without abdominal pain is not IBS.

Diarrhea without abdominal pain is a functional diarrhea (FD)

Constipation without abdominal pain is a functional constipation (FC)

Namely,

FD and IBS-D

FC and IBS-C

By comparing the population, the mechanism of occurrence of abdominal pain becomes clear.

The problem is,

FD + Factor → IBS-D

FC + Factor → IBS-C

Not only

IBS-D + Factor → FD

IBS-C + Factor → FC
