“Low-FODMAP diet” vs ”Prebiotics”

Please give the Nobel Prize to Dr. Peter Gibson in order to help the Japanese people.

Yoshiharu Uno, MD, PhD

In the world,

in the human,

"bad condition of GI tract" is

irritable bowel syndrome (IBS).

This fact is no one can deny.

Breakthrough diet of IBS is a low FODMAP diet.

In this diet, the United States, Europe and is correct in Australia.

Most Materials to avoid in IBS patients is an "oligosaccharide".

However, there is a country that is totally subjected to reverse.

It is Japan.

Japanese government agencies are recommend to intake of oligosaccharides as "can adjust the GI motility".

It is a system that has been called FOSHU.

Food for Specified Health Uses (FOSHU)
FOSHU refers to foods containing ingredient with functions for health and officially approved to claim its physiological effects on the human body.  FOSHU is intended to be consumed for the maintenance / promotion of health or special health uses by people who wish to control health conditions, including blood pressure or blood cholesterol.  In order to sell a food as FOSHU, the assessment for the safety of the food and effectiveness of the functions for health is required, and the claim must be approved by the MHLW.

Requirements for FOSHU Approval

Effectiveness on the human body is clearly proven

Absence of any safety issues (animal toxicity tests, confirmation of effects in the cases of excess intake, etc.)

Use of nutritionally appropriate ingredients (e.g. no excessive use of salt, etc.)

Guarantee of compatibility with product specifications by the time of consumption

Established quality control methods, such as specifications of products and ingredients, processes, and methods of analysis

In addition to "regular" FOSHU, following types of FOSHU are available.: Qualified FOSHU and Standardized FOSHU were introduced to facilitate applicants for FOSHU approvals.

1)      Qualified FOSHU: Food with health function which is not substantiated on scientific evidence that meets the level of FOSHU, or the food with certain effectiveness but without established mechanism of the effective element for the function will be approved as qualified FOSHU.

2)Standardized FOSHU: Standards and specifications are established for foods with sufficient FOSHU approvals and accumulation of scientific evidence.  Standardized FOSHU are approved when it meets the standards and specifications.

3)Reduction of disease risk FOSHU: Reduction of disease risk claim is permitted when reduction of disease risk is clinically and nutritionally established in an ingredient.

Most lot number of FOSHO is “helps maintain good gastrointestinal conditions.”

These are included Oligosaccharides, lactose, bifidobacteria, lactic acid bacteria, dietary fiber 8 ingestible dextrin, polydextrol, guar gum, and psyllium seed coat, etc.

The first person who proposed the FOSHU was a researcher of lactic acid bacteria.

He found that oligosaccharide increase bifidobacteria in 1982.

He proposed that "lactic acid bacteria (including bifidobacteria) is "good" bacteria.

By increasing "good" bacteria, pH in the intestines is reduced, and the short chain fatty acids (SCFA) are increased in the intestine.

And, he said.

"Reducing pH by increasing SCFA, bring to increasing peristalsis of the large intestine."

"Oligosaccharides promote the fermentation of good bacteria."

And, gradually, his story was exaggerated.

"Oligosaccharide is important for improvement of the bowel movement."

"If you suffer constipation, you must eat yogurt with oligosaccharide"

And, by the very childish research, Japan's government admitted one after another its effectiveness.

Most of the studies reported were as follow.

Self-experience

A small number of cases

Ignore the placebo effect

Conclusion of "effective for constipation" by increasing of defecation in healthy subjects

In statistically, all datas were “0.01<p”

Many were the crossover study that were not masked.

Also in the double blind study, research contents of which were known to the subjects.

And, researchers who were reported effective data were employees of sales company, or funding of the sales company.

In 2012, it was reported concerning low-FODMAP diet from the United Kingdom.

(The Journal of Nutrition.2012;142:1510-1518)

After the low-FODMAP diet, the number of bacteria, pH, and SCFA were not changed at the feces of subjects.

Concentration of bifidobacteria was only reduced.

However, concerning condition of the patient after 4 weeks of low-FODMAP diet was better than before, especially bloating and flatulence had been significantly improved.

・・・・

Recent all of the research report is......

Quite different from the dogma that story old Japanese researchers.

They had said.

After you eat a prebiotic, SCFA in gut is increased.

This is the prove that bifidobacteria has increased in your gut.

Increased bifidobacteria move your bowel.

By this way, all of your abdominal symptoms might be improved.

This dogma was Urban Legend.

However, just now, they say.
Repeat and repeat like broken stereo.

They repeat by TV commercials, feature of the magazine, internet, ….

Currently, despite scientific evidence to support this dogma has completely disappeared. . . .

In 2014, research by the capsule pH measuring instrument has been reported.

1) Gastroenterology Volume 146, Issue 5, Supplement 1, Page S-8, May 2014

2) World J Gastroenterol. 2014 May 7;20(17):5000-7. doi: 10.3748/wjg.v20.i17.5000.

By these researches, the important facts were found.

① Fermentation was carried out in the right colon.

② PH in the intestines of patients with IBS was lower as compared to healthy subjects.

③ If the pH of the cecum was low, movement of right side colon was reduced.

In other words, peristaltic motion at low pH was not enhanced.

And, fermentation simply increased the intestinal gas.

Namely, the official recognition of the Japanese government was wrong.

However,

Japanese people is difficult to change even noticed the mistake.

Already, fermented food that has received the recommendation of the government is overflowing at the store.

And, a lot of people eat it to believe can improve constipation.

Initially as was improvement in the placebo effect they feel.

But, then, their condition worse than before.

Whether there is no way to save the Japanese people?

No, this is not a Japan only problem.

This is world wide problem for the people who are brainwashed into propaganda.

Anyway, there is no way only to recognize the Low-FODMAP diet.

One way, to be recognized the correctness of the science to the world people is a Nobel Prize.

To do this,

Dr.Gibson should be awarded the Nobel Prize.

Thereby, many Japanese will notice the mistake.

And, it will improve the symptoms of 30% of people around the world.
